5th grade Immunization will take place on April 30th. *** This is for rising 6th graders only.
Forms were sent home and we strongly encourage you to take part in this clinic that is being offered by the Rockingham County Health Department here at the school. They have made this available to parents to help ease the load of taking your child this summer to get their shots. They are offering the T dap, required for 6th grade entrance and also the HPV for girls and boys. They informed me to let parents know that they are not giving shots at the fair this year Forms are required to be filled out COMPLETELY w/ Insurance Information, M'caid numbers & signatures . I am not helping with the shots, I do not give the shots, the Harrisonburg Health Dept. Nurses are. |
When to keep your child home from school:
Ways to keep you and your family healthy during the cold weather
TWO reasons why we encourage you to be sure your child dresses warm when they come to school:
1 ) Different areas of the building are cooler than others and the students like to wear their coats/jackets.
2 ) You never know when we may have to be outside for a long period of time.
- Fever of 100 degrees or higher - a fever is a sign that your body is fighting an infection
- Sore Throat/Colds/Cough/Runny Nose - Constant cough & thick yellow/green nasal drainage is a definite to stay home
- Rash - with a fever should not attend school. Rash & no fever may attend school but please notify me
- Vomiting & Diarrhea - during the night or the morning of school, please keep your child at home Thank you!
Ways to keep you and your family healthy during the cold weather
- hot meals everyday
- plenty of fluids to stay hydrated due to the dry air
- keeping your lips moistened with Carmex
- getting to bed at a descent time (warm a blanket up in the dryer & wrap it around them - mmmm, sweet dreams)
- dressing warm with coats, hats (pulled down over the ears) mittens & warm socks when going outside

Notes from the Nurse:
Thank you for your help and support throughout the year! I look forward to caring for your child!
- ALERT … I cannot accept verbal permission to give medication to your child for headache, ear ache or sore throat. Written permission from a parent or guardian is required. Please be sure to fill out a medication authorization form on page 25 for your child during registration. ( or download the one I have at the bottom of the page) If authorization is not on file, a parent must fax, email, or send written permission before a student will be given any medication from the school clinic. A new medication form must be filled out each school year.
- If you anticipate that your child may need medication during the school day, please supply that medication. All medications must be in the original container (no baggies) and small bottles are preferred.
- All medications must be brought to school by a parent or guardian. Students cannot have medication on the bus, in their backpack or purse, etc…
- If your child has a chronic medical condition (such as asthma, diabetes, seizures, bee allergy, food allergy, etc…), please speak with me prior to the start of school. Your child’s health care plan needs to be updated yearly & Mrs. Herring, our cafeteria manager, needs documentation of food allergy from the doctor.
- If your child requires a rescue inhaler, glucagon, or an Epi-pen, it is to be kept in the clinic. If your child has a medical emergency and medication is not available, 911 will be called.
- If your child is diagnosed with a contagious illness during the school year, please inform the office staff or me. This includes illnesses such as: strep throat, mono, flu, chickenpox, ringworm, pink eye, etc…
- Students are encouraged to keep an extra change of clothing in their cubby or backpack. (lunch spills, bathroom accidents, vomiting, mud puddles etc. )
- If your child requires daily snacks due to a medical condition, you are responsible to provide those snacks.
- Donations to the clinic are greatly appreciated. The following items are needed throughout the year: ginger ale, snack crackers (nabs), granola bars, juice boxes, cough drops & saltines. Clothing items: sweat pants or shorts sizes 4 – 16. T-shirts also sizes 4 – 16.
- Sneakers are a must at school. Flip flops break too easily & toes get stubbed. Please know that you will be called if your child comes to me with a broken flip flop.

Before I can administer medication to any student for a chronic illness, pain, cough or stomach ache, I need the RCPS Medication Authorization form filled out and signed by the parent/guardian. Click on the file below to print the form then drop it off to me with the medicine you would like for your child to receive, in the original container. Thank you.
